Submitting your monthly invoice to your insurance provider

Alma provides you a monthly invoice or ‘superbill’ that contains all of the information needed from your provider to submit to your insurance. Alma does not submit this claim for you. If you need additional or individual invoices we suggest reaching out to your provider. Below you can find links to reimbursement forms based on your insurance provider.

 

 

Alma is unable to advise further on how to submit for reimbursement based on specific insurance plans. As you submit for reimbursement, if you have any questions related to your specific plan, please call the phone number listed on the back of your insurance card for further assistance.
